AUSTIN, TX – May 7, 2014 – In honor of the National Nurses Week celebration this week, eMDs, a leading provider of electronic health records (EHRs) and practice management (PM) solutions, today recognized Peggy Rosen, BSN, MA, the director of quality at Mid-State Health Center, for her pioneering work in quality initiatives.
Mid-State Health Center is a Federally Qualified Health Center (FQHC) in Plymouth, NH, and also a partner in the North Country Accountable Care Organization (ACO), headquartered in Littleton, NH.
Rosen is a nationally recognized leader in healthcare for her work in advancing patient care and population health. She leads efforts to optimize the performance of the Mid-State Health Center, creating a model organization that meets the goals of the Centers for Medicare and Medicaid’s (CMS) Triple Aim quality initiative.
“Peggy Rosen has been leading the charge for years with her pioneering work on quality initiatives such as patient care, population health and Patient-Centered Medical Homes (PCMH),” said David L. Winn, MD, FAAFP, Chairman of the Board and CEO at eMDs. “Working with her highly innovative staff, Rosen helped Mid-State Health Center attain the goals of the Triple Aim before the term even became popular. eMDs is delighted and privileged to have the opportunity to work with a true innovator in Peggy. She epitomizes the very best of the nurses who work daily with eMDs solutions across the country to improve the quality of care.”
Originally developed by the Institute for Healthcare Improvement (IHI), the goals of the Triple Aim program have been adopted by the CMS. The goals are:
- Improving the patient experience of care – including quality and satisfaction
- Improving the health of populations
- Reducing the per capita cost of healthcare
“We value the power that eMDs’ Electronic Health Record offers to help us meet our quality and Triple Aim initiatives,” said Rosen. “Without a responsive EHR, we would not be able to implement new quality measures quickly. When something new comes up, we don’t have to wait six months for the next update – we have found that the eMDs EHR is flexible and robust enough to adapt to any new quality initiatives.”
Rosen, who has worked on quality issues with eMDs for the past seven years, said its EHR also enables Mid-State Health Center to translate data it needs to manage population health. Rosen works with the Mid-State team to leverage the functionality of the EHR for population health, coordinated care, and patient engagement.
“eMDs continues to be a valued partner in Mid-State Health Center’s quality initiatives,” said Rosen. “We were one of the first practices in the country to implement eMDs Patient Portal, and our patients appreciate having that virtual connection available at their convenience. It continues to be one of our key patient engagement initiatives today.”
Rosen was also the leader of Mid-State Health Center’s Patients Are Central to the Team (PACT) project. Its goal was to engage patients in playing a central role in their healthcare team by promoting increasing patient and care-giver involvement in goal-setting and care planning.
PACT has allowed Mid-State Health Center, a Level 3 PCMH, to establish a corporate culture of patient-centered care, improve patient understanding of the concept of patient-centered care, more fully engage patients as partners in their care and ultimately improve the overall patient experience. The scope and reach of Mid-State’s Patient Expert Advisory Team (PEAT), which was formed during the PACT project, has continued to grow over the past three years.
Rosen currently leads Mid-State’s Anywhere Care pilot program. Patients at locations outside Mid-State Health Center facilities will be able to access Anywhere Care’s secure videoconferencing on digital devices to meet with primary care physicians at Mid-State Health.
“With Peggy’s guidance, Mid-State Health Center has been able to significantly improve the quality of care for our patients in the communities of Southern Grafton County,” said Sharon Beaty, MBA, FACMPE, the CEO of Mid-State Health Center. “Thanks to Peggy’s efforts to help us attain the goals of the Triple Aim, our patients are more involved in their care, reducing their healthcare burdens and improving the vitality of their lives.”
